PACE of West Baltimore

PACE of West Baltimore (formerly Edenbridge PACE of West Baltimore) is a Program of All-Inclusive Care for the Elderly that helps older adults stay safe and healthy at home. An interdisciplinary team develops an individualized care plan for each participant and provides, coordinates and oversees preventive, primary, acute and long-term care. The day center offers social engagement alongside on-site medical care. Families describe participants being picked up in the morning, fed, playing games and socializing, with kind and patient drivers, and reviewers mention a warm, welcoming facility with ample accessible parking. It is licensed by the State of Maryland as an Adult Medical Day Care center for up to 60 participants.

Services include on-site physician care and medical supervision, nursing care, rehabilitation therapies, speech therapy and behavioral health, meals and nutritional counseling, and hearing, dental, vision and podiatry services. Participants receive social services and care coordination, recreational activities and exercise, social engagement at the day center, personal care and home care, pharmacy services and medication management, durable medical equipment and transportation. Respite and end-of-life care are also available as approved by the care team.

The center is at 3201 Tioga Pkwy Ste A, Baltimore, MD 21215, and is open Monday through Friday, 8 a.m. to 5 p.m. To enroll, participants must be 55 or older, live in the PACE service area, need a nursing-facility level of care, be able to live safely in the community with PACE support, and agree to receive all care through PACE.
