Element Care PACE

Element Care PACE is dedicated to providing complete and highly personalized care to seniors, enabling them to live independently, safely, and comfortably in their homes for as long as possible. The program focuses on promoting wellness, engagement, and purposeful living through a variety of activities and social opportunities. Participants can choose from tai chi, music and art programs, cultural events, cooking classes, and wellness gyms. The PACE program has been shown to help participants live longer, healthier lives with fewer hospitalizations and unmet needs. A caring team of doctors, nurses, therapists, and other professionals works together to ensure comprehensive care for all participants.

Element Care PACE offers a wide range of services designed to meet the health and wellness goals of seniors. Activities include tai chi, music and art programs, cultural events, cooking classes, and wellness gyms. The program also features annual Olympics and other engaging events. Participants receive care from a dedicated team of healthcare professionals, including doctors, nurses, home health aides, physical and occupational therapists, behavioral health clinicians, social workers, and recreational specialists. This team meets daily to discuss any changes in participants' health, ensuring quick responses and holistic care. The program also integrates innovative technology and telehealth services to enhance continuity of care.

Element Care PACE is located at 100 Cummings Center # 166D, Beverly, MA 01915. The program provides transportation services for participants and accepts Medicaid. For those who do not qualify for Medicaid, there are options to cover the monthly premium. Free interpreter services are available by calling 1-781-715-6612 (TTY: 711).
