Teamcare, a program of CenterLight Healthcare, is dedicated to providing comprehensive adult day care services in Brooklyn. As part of the largest not-for-profit Program of All-Inclusive Care for the Elderly (PACE) in the nation, Teamcare offers a wide range of services designed to help seniors live independently at home. Their interdisciplinary team includes physicians, nurses, social workers, and therapists who work together to create personalized care plans. Participants can enjoy nutritious meals, medical care, and a variety of therapeutic activities at their day centers. CenterLight Healthcare PACE is committed to ensuring participants receive the care they need while maintaining their independence and quality of life.

Teamcare provides a holistic approach to adult day care, offering medical care, social support, and therapeutic activities. Medical services include primary and specialty care, dental, podiatry, and eye care, as well as rehabilitation services like physical and occupational therapies. Nutritional counseling and laboratory services are also available. Social activities are a key component, with options such as art therapy, music therapy, dance and movement therapy, and pet therapy. Participants can also engage in games, cognitive challenges, and cultural events. The program emphasizes personalized care, with a dedicated team working to meet each participant's unique needs. Transportation to and from the day center and medical appointments is provided, ensuring accessibility for all participants.

Teamcare is located at 205 Rockaway Pkwy, Brooklyn, NY 11212. The program accepts Medicaid and provides transportation services to participants. TTY users should call 711. The program is designed for individuals 55 and older who are Medicaid eligible or entitled to Medicare Part A and/or enrolled in Medicare Part B. Participants must live in the service area and need nursing home level of care but are able to remain living safely at home. CenterLight Healthcare PACE aims to make enrollment as seamless as possible, with a team ready to assist with any questions or needs.
