Hope PACE

Hope PACE is a unique program designed to help seniors live independently in their homes by providing comprehensive all-inclusive care. Unlike traditional nursing homes, Hope PACE focuses on delivering primary and specialty medical care, medications, homecare, and socialization services directly to participants. The program is ideal for those at risk of needing nursing home care but wish to remain in their communities. Hope PACE serves residents of Charlotte, Collier, and Lee counties who meet eligibility criteria, including being at least 55 years old and able to live safely in the community. The program is free for Medicaid participants, with fees applicable for those with Medicare only or private pay. Hope PACE emphasizes a team-based approach, involving primary care doctors, nurses, social workers, and therapists to ensure holistic care.

Hope PACE provides a wide range of services to support seniors in living independently. These include primary medical and nursing care, all necessary prescription drugs, lab and diagnostic services, and therapies such as occupational, physical, and speech therapy. Home care and personal care aides are available to assist with daily activities, and hospitalization and skilled nursing facility care are covered when needed. The program also offers respite services to alleviate caregiver stress. A dedicated Care Team, including primary care doctors, nurses, social workers, and therapists, collaborates to meet each participant's unique needs. Medical specialists are available for additional care, with services pre-approved by the Care Team. Hope PACE ensures all necessary medications, including prescription and over-the-counter drugs, are covered, eliminating the need for separate Medicare Part D plans.

Hope PACE is located at 1435 Santa Barbara Blvd, Cape Coral, FL, and serves Charlotte, Collier, and Lee counties. The program operates Monday through Friday from 8:00 a.m. to 5:00 p.m., closing on major holidays. Transportation is provided for participants, ensuring access to care and services. The program is free for those with Medicaid coverage, while fees apply for Medicare-only or private pay participants. Hope PACE assists with Medicaid eligibility explanations and cost estimates for those without dual coverage.
