Home Avenue Adult Day Health Care

Home Avenue Adult Day Health Care (ADHC/CBAS) has served families in Los Angeles County and parts of Orange County since 2003. It serves adults 18 and older with chronic medical, cognitive or mental health conditions such as diabetes, dementia, hypertension, depression or anxiety who need help with daily activities and are at risk of hospitalization or institutionalization. The center also offers CalAIM Enhanced Care Management in partnership with L.A. Care and a free educational series for caregivers. Reviewers praise the nurses and activities, while one raised concerns about COVID-19 handling. It is licensed by the California Department of Public Health as an Adult Day Health Care center.

Services include an individual needs assessment, nursing care, physical and occupational therapy, daily maintenance exercises, psychiatric and psychological services, podiatry, social services and family consultation. Participants receive hot meals and snacks and take part in therapeutic activities and community outings. Staff arrange transportation between home and the center.

Home Avenue is at 8114 Telegraph Rd, Downey, CA 90240, and is open Monday through Friday, 9:00 AM to 5:00 PM. CBAS is covered by Medi-Cal (including plans such as L.A. Care, Health Net, Molina, Anthem Blue Cross, CalOptima and Kaiser Permanente) or private pay; the center obtains prior authorization from the participant's plan.
