Senior CommUnity Care of North Carolina PACE is a Program of All-Inclusive Care for the Elderly designed to help seniors live independently at home with a high quality of life. The program serves seniors in select counties of Wake, Durham, and Granville, providing comprehensive health care and social services. Participants receive coordinated care from a multidisciplinary team, including primary and specialty medical care, prescription medications, and home care services. The program aims to prevent nursing home placement by offering supportive services in the community.
