Senior CommUnity Care of North Carolina PACE is a Program of All-Inclusive Care for the Elderly designed to help seniors live independently at home with a high quality of life. The program serves seniors in select counties of Wake, Durham, and Granville, providing comprehensive health care and social services. Participants receive coordinated care from a multidisciplinary team, including primary and specialty medical care, prescription medications, and home care services. The program aims to prevent nursing home placement by offering supportive services in the community. PACE is ideal for seniors who need nursing-level care but wish to remain at home, as well as their caregivers and health professionals supporting elder care.

Senior CommUnity Care of North Carolina PACE offers a wide range of services tailored to the needs of each participant. These include primary and specialty medical care, prescription medications, adult day services, and home care assistance. Mental health and social services are available, along with skilled therapies such as physical and occupational therapy. The program also covers hospital and nursing home care when necessary. Transportation is provided to and from the PACE Center and medical appointments. Participants receive personalized care plans developed by an interdisciplinary team, ensuring all health and social needs are met. The program supports caregivers with training, support groups, and respite care.

Transportation services are available for participants to attend the PACE Center and medical appointments. Eligibility requires participants to be at least 55 years old, certified for nursing home care, and able to live safely in the community.
