Fontana Adult Day Health Services is a Community Based Adult Service/Adult Day Health Care (CBAS/ADHC) program designed to support adults aged 18 and older with chronic medical, cognitive, or mental health conditions. The center aims to promote active living and healthy aging while reducing social isolation and preventing premature nursing home placement. With a focus on safety, dignity, and respect, Fontana ADHS provides a stimulating and professional care environment. The center offers affordable care, with full services costing $0 to qualified Medi-Cal Managed Care recipients. Private pay rates are significantly lower than most in-home care options in California. Fontana ADHS collaborates with family members, primary care physicians, and other care specialists to create a person-centered community support system, ensuring comprehensive care for all members.
