Broward PACE Program

Broward PACE Program is a collaboration between Broward Health and Florida PACE Centers by Miami Jewish Health, designed to help seniors avoid nursing home care by providing personalized healthcare and support services. The program serves seniors in central and northern Broward County, offering a comprehensive range of services tailored to individual needs. Participants receive coordinated care, including medical appointments, medications, and adult day care with transportation, meals, and activities. The program emphasizes maintaining health and quality of life through individualized care plans developed with participants and their families.

Broward PACE Program offers a wide array of services to meet the healthcare and social needs of seniors. These include primary and specialty medical care, prescriptions, adult day care, dental care, meals, nutritional counseling, social work, personal care, home care, medical equipment, transportation, and various therapies. The program ensures 24/7 access to healthcare services, with urgent care needs addressed promptly. Participants receive care from a dedicated medical team, including a primary care physician and specialists. Transportation is provided for appointments and center visits, and meals and snacks are available during adult day care hours. The program also covers hospital and nursing home care when necessary.

Broward PACE Program is located at 1700 NW 49th St Suite 150, Fort Lauderdale, FL 33309. The program accepts Medicaid and Medicare, with costs fully covered for eligible participants. Private pay options are also available. Transportation is provided for participants, ensuring safe door-to-door travel to and from the center and healthcare appointments. The adult day health centers are open from 8am to 4pm, Monday through Friday, offering breakfast, lunch, activities, and coordinated care. For emergencies, participants are advised to call 911 or go to the nearest Emergency Room, then contact the program for authorization. The program is committed to accessibility and provides support for individuals with disabilities.
