Trinity Health PACE Montgomery County

Trinity Health PACE Montgomery County is a Program of All-Inclusive Care for the Elderly (PACE) on Perry Parkway in Gaithersburg. It helps older adults who need significant care continue living at home by coordinating medical care, therapies, social services and daytime activities through one interdisciplinary team at the PACE center. The center's clinic becomes the participant's primary care provider, and participants also come for meals, recreational therapy and social time. It is licensed in Maryland as an Adult Medical Day Care center for up to 104 participants.

Physicians, nurse practitioners and specialists at the on-site clinic provide primary, nursing and specialty care, medications, and dental, vision, hearing and foot care. Physical, occupational and speech therapy and recreational therapy (arts and crafts, chair yoga, computer classes, bingo, trivia, day trips) are offered at the center, along with breakfast and lunch. Wheelchair-friendly vans provide free transportation to the center and medical appointments, and personal care aides can help at home.

Trinity Health PACE Montgomery County is located at 200 Perry Pkwy Suite 1, Gaithersburg, MD 20877, and is open Monday through Friday from 8:00 AM to 4:30 PM.
