Prisma Health SeniorCare PACE Greenville

Prisma Health SeniorCare PACE is a Program of All-Inclusive Care for the Elderly run by Prisma Health, a nonprofit health system. The program helps participants stay in their own homes while receiving care comparable to a nursing home level, coordinated by an interdisciplinary team that includes physicians, nurses, therapists, social workers, recreational therapists, pharmacists and a transportation team. A clinic provider is on site at the Greenville PACE center every day, and participants also take part in activities and build friendships there. It is licensed in South Carolina as an adult day care facility for up to 140 participants.
