The PACE Place

The PACE Place is a Program of All-Inclusive Care for the Elderly (PACE) with a day center on Ramona Boulevard in Jacksonville. Its team of doctors, nurses, therapists and certified nursing assistants plans care with each participant and family, providing services at the day center, at home and through a provider network. It is for adults 55 and older who live in its Jacksonville-area service zip codes, can live safely in the community and are approved by the state for nursing home level of care. Some families praise the help with appointments, therapy, meals and activities, while several reviewers describe delays and poor communication with families. It is licensed by the State of Florida as an Adult Day Care Center for up to 114 participants.

The plan covers primary and specialty medical care, prescription medications, dental, vision, hearing and foot care, lab work and X-rays, and emergency and hospital care. Physical, occupational and speech therapy, home medical equipment, in-home personal care and nursing support, light housekeeping and respite care are included. Participants get transportation to the center and appointments, along with recreation, meals and social time at the day center, and ongoing care coordination and social work support.

The PACE Place is at 5450 Ramona Blvd, Jacksonville, FL 32205, and is open Monday through Friday from 8 a.m. to 5 p.m. There is no cost to join for people with both Medicare and Medicaid or Medicaid alone; people with Medicare only or neither can join for a monthly fee, and private pay is welcome.
