Senior CommUnity Care of Michigan PACE is a comprehensive health care program designed for seniors who wish to live independently but require support. Services & Care
Senior CommUnity Care of Michigan PACE offers a wide array of services to meet the needs of its participants. The Adult Day Health Center provides activities such as bingo, crafts, trivia, and a walking club, along with a Memory Lounge for those needing more individualized care. Participants receive medical and specialty care, including primary care, nursing, dental, vision, and podiatry services. Transportation is provided to and from the PACE Center and medical appointments. Nutritious meals are served on weekdays, with take-home options available. Therapy services, including physical and occupational therapy, are offered to improve mobility and daily living skills. Home health and personal care services, such as bathing and meal preparation, are also available. The program covers prescription and over-the-counter medications, and respite services are provided for caregivers.

The program accepts Medicaid and helps determine eligibility for participants. Transportation services are provided to ensure participants can attend the center and medical appointments. Enrollment is open to individuals aged 55 or older who meet the state's criteria for nursing home level of care and can live safely in the community with PACE services.
