Silver Star Health Network

Silver Star Health Network runs a PACE (Program of All-Inclusive Care for the Elderly) program in Lubbock for people 55 and older who need a nursing-home level of care but can live safely at home. Participants attend the Silver Star Day Health and Wellness Center, which has a fully staffed medical clinic, therapy, exercise equipment and social and recreational activities, while an interdisciplinary team coordinates all of their medical care. Reviews are mixed: some families praise the van drivers, on-site therapy and attentive staff, while others report frustration with physician care and after-hours response. It is licensed by Texas Health and Human Services as a Day Activity and Health Services facility for up to 138 participants.

The day health and wellness center offers a fully staffed medical clinic, physical, occupational and speech therapy, exercise equipment, an outdoor patio and gardening area, social and recreational activities, and a hot lunch and snacks, with transportation to and from the center. Through PACE, participants also receive physician services, prescription medications, hospital care, skilled nursing, home health care, social services and rides to medical appointments, with prior authorization from the interdisciplinary team. Caregivers can get training, support groups and respite.

Silver Star is located at 4010 22nd St, Lubbock, TX 79410, and is open Monday through Friday from 8 a.m. to 5 p.m. Participants must be 55 or older, live in Lubbock County, need skilled nursing services and be able to live safely in the community. Costs are fully covered for people enrolled in both Medicare and Medicaid; Medicare-only and private pay options are also available, and staff help with financial eligibility.
