Central Valley PACE

Central Valley PACE is a Program of All-Inclusive Care for the Elderly (PACE) designed to help seniors aged 55 and older who qualify for nursing facility level of care. The program offers a comprehensive range of services, including medical, dental, and vision care, as well as therapeutic activities and home care assistance. Participants can enjoy a variety of recreational activities such as music, karaoke, pool, and arts and crafts in a bright and welcoming social center. The program also provides transportation services for off-site medical appointments and a free healthy lunch. Central Valley PACE is committed to personalized care, ensuring each participant receives a tailored plan to meet their unique needs.

Central Valley PACE provides a full spectrum of medical and long-term care services, including comprehensive medical, dental, and vision care. Participants benefit from personalized preventive care and chronic condition management. The adult day program features therapeutic recreation, physical and occupational therapies, and activities like music, karaoke, pool, and arts and crafts. Home care services are available, with medically trained staff visiting as often as needed. Nutritional counseling, meals, and transportation services are also provided. The program ensures all services are tailored to the participant's physical and psychosocial needs, as determined by their interdisciplinary care team.

Central Valley PACE is located at 2401 E Orangeburg Ave #330, Modesto, CA 95355, with additional locations in Merced. The program operates Monday to Friday from 8:00 a.m. to 4:30 p.m. Transportation services are available for medical appointments, using handicap-accessible vehicles. Central Valley PACE accepts Medicaid and is funded by Medicare and Medicaid, with most participants paying nothing additional for the program. Language assistance services are available for non-English speakers. Participants must reside within the Central Valley PACE service area to qualify for services.
