Hong Fook Center

Hong Fook Center is a Community-Based Adult Services (CBAS) facility located in Oakland, CA, serving frail adults over 18 years of age who are medically eligible. The center provides a range of social, health, and therapeutic services designed to support individuals with conditions such as high blood pressure, diabetes, stroke, cardiovascular conditions, and dementia. Enrollment involves a visit to the center, a three-day assessment, and a care plan development.
