DayBreak Senior Services is Northern Utah's only licensed Adult Day Health Program, offering a dementia-informed and person-centered approach to care. The facility has received funding to enhance its dementia-specific services, including the creation of a therapeutic sensory garden and the addition of a full-time Recreational Therapist. DayBreak's goal is to offer an affordable care option that enables older adults to remain at home, supported by a community that values their individuality and needs.

DayBreak Senior Services specializes in memory care, offering a range of activities designed to support the five domains of health: cognitive, physical, emotional, social, and spiritual. Participants engage in cognitive activities like games and trivia, physical movement through chair exercises and walking groups, emotional wellness activities such as art and music, social connections via group games and shared meals, and spiritual opportunities including gratitude circles. The program is led by a Certified Therapeutic Recreation Specialist, ensuring activities are tailored to each participant's abilities and preferences. DayBreak also provides meals, snacks, and individualized care, with staff trained to assist with personal care, mobility, and incontinence support, always prioritizing dignity and independence. The facility is not a medical facility but maintains health information for participant safety.

DayBreak Senior Services is located at 1351 Valley Dr, Ogden, UT 84401, and operates Monday through Friday from 9 a.m. to 6 p.m. The program accepts Medicaid, private pay, and offers funding options through partnerships with the Veterans Administration and local caregiver support programs. Private pay rates start at $99/day for 1-4 days per week, with a reduced introductory rate of $74/day for the first month. Transportation is provided within Davis and Weber Counties, and the facility is closed on major holidays. DayBreak encourages prospective participants to complete a 'Get to Know Me' profile and schedule a tour to ensure the program meets their needs. The facility is designed to be dementia-friendly, with recent upgrades enhancing its engaging and supportive environment.
