The Oaks PACE

The Oaks PACE, part of the Orangeburg Senior Helping Center, is dedicated to enhancing the quality of life for frail elderly adults in Orangeburg, Bamberg, and Calhoun Counties. The program offers a comprehensive, coordinated healthcare solution under the PACE model, which stands for Program of All-Inclusive Care for the Elderly. This model is designed to deliver all necessary medical and supportive services to seniors with chronic care needs, allowing them to remain in their communities rather than moving to a nursing home. The center operates with a team approach, involving a diverse, interdisciplinary team of medical and support service experts. Participants receive personalized care plans that address their specific needs, including primary care, recreational therapy, nutrition, and more. The Oaks PACE provides a wide range of services tailored to the needs of each participant. Additional services such as audiology, dentistry, optometry, podiatry, and speech therapy are also available as needed. The Oaks PACE is located at 153 Founders Court, Orangeburg, SC 29118. The center is open Monday to Friday from 8:00 am to 5:00 pm. Eligibility requires individuals to be 55 or older, certified by the State of South Carolina to need nursing home level of care, and residents of Orangeburg, Bamberg, or Calhoun counties. Participants must sign an Enrollment Agreement with PACE and receive all healthcare services through the program. Medicaid-eligible individuals pay no monthly premium, while those without Medicaid may enroll as private pay participants.
