Trinity Health PACE of Pensacola

Trinity Health PACE of Pensacola is a PACE (Program of All-Inclusive Care for the Elderly) center, part of Trinity Health, serving older adults in the Pensacola area. An interdisciplinary team of primary care providers, nurses, therapists, social workers and dietitians coordinates each participant's care at the center. Participants also take part in recreational therapy such as arts and crafts, computer classes, gentle movement, pet therapy, bingo and celebrations. Transportation to the center and to medical appointments is included at no cost. It is licensed by the State of Florida (Agency for Health Care Administration) as an Adult Day Care Center (license 9571) for up to 250 participants.

Medical services include primary care, nursing, specialty care, medications, vision, hearing, dental and foot care, and mental health services. Physical, occupational and speech therapists work with participants to maintain strength and independence, and recreational therapy includes arts and crafts, computer classes, gentle movement, pet therapy, bingo and trivia. A registered dietitian plans nutrition, social workers support participants and families, and trained drivers provide transportation to the center and appointments.
