Wisconsin Adult Center

Wisconsin Adult Center is dedicated to improving the quality of life for adults aged 18 and older who have cognitive or physical disabilities, traumatic brain injuries, or are of advanced age. Their member-centered approach ensures that each individual's needs and preferences guide the program's activities and structure. Wisconsin Adult Center is located at 1275 E Wisconsin Ave, Pewaukee, WI 53072, and operates Monday through Friday from 8:00am to 3:00pm. The center is closed on major holidays such as New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas. They accept various family care programs including Mychoice, Community Care, I-Care, Inclusa, and Lakeland Care, as well as IRIS and private pay options. Prior to enrollment, potential members are encouraged to complete a tour and assessment forms. Trial days are available to assess fit but require authorization from the MCO or budget team, or advance payment for private pay. The center works with schools to help students transition smoothly into the program, starting as early as age 18 during school breaks and summer.
