Piedmont Health SeniorCare – Pittsboro PACE Center

Piedmont Health SeniorCare's Pittsboro center is a Program of All-Inclusive Care for the Elderly (PACE) run by Piedmont Health Services, a community health center network. It serves adults 55 and older who are eligible for nursing home care but can live safely at home with support. Participants attend the day health center, where a team of doctors, nurses, therapists, social workers, a pharmacist and dietitians coordinates their care. Families describe staff who kept a husband with dementia engaged at the day center and stayed with him as his needs grew, though one participant disliked the limits on outings and food choices. It is certified by North Carolina as an Adult Day Health program for up to 120 participants.

Services include primary care, nursing, physical, occupational and recreational therapy, medication management, prescriptions, dental care, nutrition counseling, social services, mental health services and personal care at the center and at home. Meals and snacks are served at the day center, and transportation is provided to the center and to medical appointments. Respite care and caregiver education are also offered.

The center is at 163 Chatham Business Dr, Pittsboro, NC 27312, and is open Monday through Friday from 8 a.m. to 5 p.m., with on-call services around the clock. Participants must be 55 or older, live in Alamance, Caswell, Chatham, Lee or Orange County (or ZIP code 27278), be nursing-home eligible and able to live safely in the community; people with Medicare only pay a fixed monthly premium, and private pay is possible.
