Queens Community House Social Adult Day Services
108-25 62nd Dr, Forest Hills, NY 11375
Queens Community House (QCH), a Queens nonprofit, offers Social Adult Day Services at its Forest Hills Community Center on 62nd Drive. The program serves older adults who live at home but are physically frail, socially isolated or memory-impaired, and it gives family caregivers respite. QCH runs two program models: LIFT, for people who are frail or have mild memory impairment, and TYME AWAY, for people living with moderate to early late-stage Alzheimer's disease or related dementia. QCH has more than 30 years of experience running social day care. It is listed in New York City's registry of social adult day care programs.
Services & Care
Participants take part in supervised educational and recreational activities with trained staff. A hot, balanced lunch and snacks are served daily, and field trips and special events are planned through the year. Transportation, case assistance and other support services are available as needed. The TYME AWAY model is designed for people with moderate to early late-stage Alzheimer's or related dementia.
