On Lok PACE Gee Center

The On Lok PACE Gee Center is one of On Lok's PACE (Program of All-Inclusive Care for the Elderly) centers, serving older adults across San Francisco County who want to keep living at home. An interdisciplinary care team coordinates health care, personal care and support services, and the center includes a medical clinic and rehabilitation gym. Participants come for day center activities, social connection and meals, with support available in multiple languages.
