Gary and Mary West PACE

Gary and Mary West PACE is a nonprofit PACE program, serving North County San Diego since 2019, for residents who qualify for nursing home-level care but want to keep living at home. It combines Medicare and Medi-Cal coverage with an adult day care and health center in San Marcos, in-home and outpatient rehabilitation, scheduled transportation, meals and nutrition support, medication management and telehealth. At the day center, participants share meals and snacks and take part in activities such as pet therapy, bingo, yoga and chair tai chi. Families describe responsive, caring staff and say having all health and social needs in one place made care easier, while other reviewers report slow referrals and authorizations. It is licensed by the California Department of Public Health as an Adult Day Health Care center.

The PACE team provides medical care, in-home and outpatient rehabilitation, medication management, telehealth and home services. At the San Marcos day center, participants enjoy daily social activities with meals and snacks, pet therapy, bingo and group fitness such as yoga and chair tai chi. Scheduled transportation brings participants to the center and appointments.

The center is at 1706 Descanso Ave, San Marcos, CA 92078. Hours are Monday through Friday, 8:00 a.m. to 4:30 p.m. The program serves North County San Diego residents who qualify for nursing home-level care; costs depend on insurance coverage, and many members pay nothing.
