Day Break at The Hub on Smith

Day Break is the adult day services program of The Hub on Smith, the senior center in Sheridan, Wyoming. It offers person-centered activities and gives family caregivers a break, helping people stay at home and out of long-term care. Each participant has a customized plan of care, and staff assist with toileting, bathing, feeding, walking and medication reminders. Participants receive snacks and a full noon meal at The Hub's congregate meal site. It is licensed by the Wyoming Department of Health as an adult day care.

Staff help with daily living tasks such as toileting, bathing, feeding and walking according to each care plan, and provide medication reminders and mealtime assistance. A bathing facility with a walk-in accessible shower and hydrotherapy tub is available. Mid-morning and afternoon snacks and a full noon meal are provided, with dietary needs accommodated. Goose Creek Transit may provide door-to-door transportation.

Day Break is at The Hub on Smith, 241 Smith St, Sheridan, WY 82801, and is open weekdays from 8:00 AM to 4:00 PM. Schedules are set at enrollment and can be changed, with 24 hours' notice for extra days or cancellations; a TB test is required for new participants.
