Milton Adult Day Services, located in South Bend, IN, provides a safe and compassionate environment for adults who need assistance during the day. The facility offers a home-like setting with specialized memory care services, designed to meet the individual needs of functionally impaired adults. Milton Village, a collaboration between Milton Adult Day Services and Alzheimer’s & Dementia Services of Northern Indiana, is the first of its kind in the United States, based on the Netherlands' de Hogeweyk model. This revolutionary concept focuses on providing a town-like setting that promotes independence and reduces social isolation. The program includes a variety of engaging activities and holistic care to enhance the quality of life for participants.

Milton Adult Day Services offers a structured day program with engaging activities designed to maintain or enhance function. These include exercise, arts, baking club, billiards, and crafts. The facility provides a light breakfast, hot lunch, and an afternoon snack. Specialized memory care services are available, along with personalized care planning and health monitoring. The environment is designed to be safe and home-like, promoting independence and dignity. Staff are trained to provide compassionate care and support for both guests and caregivers. Additional services such as showers and shaves are available for an extra fee.

Milton Adult Day Services is located at 111 Sunnybrook Ct, South Bend, IN 46637. The facility is open Monday through Friday from 7:30 a.m. to 5:00 p.m. and is closed on major holidays including New Year’s Day, Memorial Day, 4th of July, Labor Day, Thanksgiving Day, and Christmas Day. The cost is $75.00 for up to five hours of attendance, with additional hours billed at $15.00 per hour. Financial assistance is available for those who qualify, including veterans and Medicaid waiver recipients. Enrollment requires an interview, physician’s order form, and tuberculosis testing. Guests are encouraged to attend at least two half days per week to establish a routine and build relationships with staff and other guests.
