Huron Valley PACE is a Program of All-Inclusive Care for the Elderly designed to help seniors remain independent in their communities. It offers a comprehensive package of medical, wellness, and support services tailored to individual needs. The program includes access to a medical clinic, specialists, prescription drugs, and emergency care, all coordinated by an interdisciplinary team. Participants can enjoy social activities, nutritious meals, and educational classes at the adult day center. The service area covers Washtenaw and Monroe counties, as well as parts of Oakland, Wayne, and Livingston counties. Eligibility requires being 55 or older, living in the service area, and needing nursing home-level care.

Huron Valley PACE manages every aspect of care for seniors, including medical services with physicians and nurses specializing in older adults, all necessary prescription drugs, and emergency care. Wellness services encompass homemaker assistance, recreation therapy, counseling, and medication management. Physical, occupational, and speech therapies are also available. Social services include an adult day center with meals, recreational activities, monthly outings, and educational classes. Support services feature medical transportation, case management, personal care, laundry services, respite care, and caregiver support. Huron Valley PACE is located at 2940 Ellsworth Road in Ypsilanti, MI. The program accepts Medicaid and helps determine eligibility for financial assistance. Transportation is provided between home and the day center, and for some medical appointments. Enrollment involves a home visit, a day center visit for assessments, and signing an enrollment agreement. Services begin on the first of the month following enrollment.
