Day Break Adult Day Health Care Center, located at the Bloss Memorial Center in Atwater, CA, serves the elderly and adults with physical and mental impairments. Day Break provides a range of services to support its participants. Nutritious hot meals and snacks are served daily, and medication management is available with on-site emergency treatment. The social work department helps locate community resources and offers psychosocial services, including psychotherapy by Marriage and Family Therapists and Licensed Clinical Social Workers. Activities include arts and crafts, movie days, and bingo games to keep minds and bodies active. Physical and occupational therapy are available to maintain health and wellness. Transportation is provided between residences and the program, covering areas from Atwater to Livingston. Wheelchair-bound individuals who qualify must use para-transit.

Day Break Adult Day Health Care Center is located at 1251 Grove Ave Suite E, Atwater, CA 95301. The center operates Monday through Friday, with participants able to attend one to five days a week from 8:00 AM to 12:00 PM. Transportation services are available for participants. Costs may be covered by Medi-Cal or private pay. Those not covered by these options should contact the Day Break social worker to discuss payment. Approval for services through insurance may take up to three weeks. The center accepts donations of non-perishable food items, clothing, walkers, wheelchairs, and adult diapers.
