Piedmont Health SeniorCare PACE – Burlington

Piedmont Health SeniorCare's Burlington center on Vaughn Road is a Program of All-Inclusive Care for the Elderly (PACE) run by Piedmont Health Services, a community health center network. It provides complete care for older adults who are eligible for a nursing home but want to keep living at home, with medical care, prescriptions, therapy, meals and transportation coordinated by one team at an adult day health center.
