SSM Health Adult Day Services is a program of SSM Health St. Agnes Hospital, located near the hospital on East Merrill Avenue in Fond du Lac. It serves older adults, people with chronic illness and adults with disabilities, including those recovering from hospital care or needing supervision while a caregiver works or takes a break. The program is supervised by a registered nurse, with care provided by licensed practical nurses, certified nursing assistants and an activities coordinator. Participants may attend full or half days, one to five days a week. It is certified by the Wisconsin Department of Health Services as an Adult Day Care Center (ADCC) for up to 40 participants.
Services & Care
The daily fee covers recreational and social activities, breakfast, a hot noon meal and an afternoon snack, help with activities of daily living, group and individual exercise, medication supervision and administration, basic health monitoring and nursing care ordered by a provider. Bathing services use two whirlpool tubs with hydraulic seats, with hair and nail care included. Daily activities include cards, games, exercise and community outings.
