PACE of the Triad

PACE of the Triad is a Program of All-inclusive Care for the Elderly (PACE) designed to provide community-based services for older adults who need medical care and support to continue living at home. The program serves individuals in Guilford and Rockingham counties, offering a holistic approach to healthcare that includes medical, dental, vision, and hearing services. PACE emphasizes personalized care, ensuring each participant receives attention tailored to their unique needs. The program also fosters independence and social connections through activities like fitness classes, outings, and games, all within a supportive and welcoming environment. With a focus on preventing hospitalizations and emergency room visits, PACE aims to enhance the quality of life for seniors by addressing their health and social needs comprehensively.

Meals, including breakfast and lunch provided by Golden Corral, are part of the daily routine. Transportation to medical appointments and the PACE center is provided, making it easier for participants to access care and social activities. Specialized memory care services are available for those with dementia or Alzheimer's, and home care services ensure continuous support for those living independently.

PACE of the Triad is located at 1471 E Cone Blvd, Greensboro, NC 27405. The program accepts Medicaid, and transportation services are provided for participants. For those without Medicaid or Medicare, a monthly premium may apply based on individual financial and healthcare needs. Enrollment is open to individuals aged 55 or older who live in Guilford or Rockingham counties and can live safely in the community with support. To enroll or learn more, contact PACE at (336) 550-4040. The program does not have a waiting list, and participants can dis-enroll at any time. PACE operates as both the insurer and healthcare provider, eliminating the need for additional private insurance.
