PACE Your LIFE is a Program of All-Inclusive Care for the Elderly (PACE) designed to help seniors aged 55 and older maintain their independence while receiving comprehensive medical and social services. The program is ideal for those who require assistance but wish to remain in their homes. A dedicated team of healthcare professionals works with each participant to create a personalized care plan that addresses their specific needs. Services are primarily delivered at the PACE Day Center, with additional support available in participants' homes or through the PACE Your LIFE community network. The program emphasizes improving and maintaining health, ensuring participants can live independently for as long as possible.

PACE Your LIFE offers a wide range of services tailored to meet the needs of its participants. These include on-site physician and nursing care, physical and occupational therapy, recreational therapy, and nutritional counseling. Participants also benefit from meal services, transportation to and from the Day Center, and assistance with daily needs such as bill payments and home management. The program provides 24/7 medical support and emergency services, ensuring comprehensive care at all times. All services are coordinated by an interdisciplinary team that works closely with participants and their families to develop and implement effective care plans.

PACE Your LIFE is located at 21 West Clarke Avenue, Suite 1010, Milford, DE 19963. The Day Center operates Monday through Friday from 8:00 AM to 5:00 PM, with on-call services available evenings and weekends.
