Immanuel Pathways PACE – Eastern Nebraska

Immanuel Pathways Eastern Nebraska is a Program of All-Inclusive Care for the Elderly (PACE) for adults 55 and older who need a nursing home level of care but wish to keep living at home. A dedicated care team coordinates each participant's care across their home and the Immanuel Pathways Center on Sorensen Parkway, including adult day services with meals, personal care and social work. PACE acts as both health care provider and health plan, and most participants are dually eligible for Medicare and Medicaid. Reviews are mixed, with some participants saying they are well cared for and others raising concerns about management. It is licensed by the State of Nebraska as an Adult Day Service for up to 160 participants.

PACE covers primary and specialty medical care (including audiology, dentistry, optometry and podiatry), prescriptions, home care, respite care, therapies and transportation. Adult day services at the center include meals, nutritional counseling, social work and personal care. Hospital and nursing home care are covered when necessary, along with all Medicare and Medicaid-covered benefits.

The center is at 5755 Sorensen Pkwy, Omaha, NE 68152, and is open Monday through Friday, 7 a.m. to 5 p.m. It serves adults 55 and older in select ZIP codes of Douglas, Sarpy, Cass, Dodge, Saunders and Washington counties who meet nursing home level of care.
