San Diego PACE San Ysidro

San Diego PACE, a division of San Ysidro Health, is a Program of All-Inclusive Care for the Elderly for adults 55 and older who meet nursing facility level of care and can live safely at home with support. The San Ysidro center offers an adult day center along with primary care, nursing, rehabilitation therapy, behavioral health, prescriptions and social services, coordinated by a bilingual team of geriatric specialists. Several reviewers report problems with communication, unreturned calls and form delays. It is licensed by the California Department of Public Health as an Adult Day Health Care center.

PACE benefits include the adult day center, primary medical and specialty care, nursing care, rehabilitation therapy, behavioral health, prescription medications, durable medical equipment and social services. Care is coordinated by an interdisciplinary team and can be delivered at the center or in the home.

San Diego PACE San Ysidro is at 3364 Beyer Blvd, San Ysidro, CA 92173, open Monday through Friday, 8 a.m. to 5 p.m. Enrollees must be 55 or older, live in the service area and meet nursing facility level of care. PACE is funded by Medicare and Medi-Cal; there are no out-of-pocket costs for those eligible for Medi-Cal, and others pay a monthly premium.
