On Lok PACE 30th Street Center

The On Lok PACE 30th Street Center is one of On Lok's PACE (Program of All-Inclusive Care for the Elderly) centers, serving older adults throughout San Francisco County so they can keep living at home. An interdisciplinary care team coordinates each participant's health care, personal care and support services, and like other On Lok PACE centers it has an on-site clinic and rehabilitation gym. Participants spend time at the day center for activities, socializing and meals, and On Lok offers support in multiple languages. The building also houses On Lok's 30th Street Senior Center, and reviewers of the senior center mention meals, fitness classes, a gym and a garden. It is licensed by the California Department of Public Health as an Adult Day Health Care center.

Covered services include primary care, dental care, physical therapy, vision and hearing services and home health care, coordinated by an interdisciplinary team. The center has a medical clinic and a rehabilitation gym on site. Day center activities across On Lok's centers include exercise and dance, music and art classes, cultural celebrations and community meals, and transportation is provided to medical appointments and center visits.
