Ascension Living Alexian PACE

Alexian PACE is a Program of All-Inclusive Care for the Elderly run by Ascension Living for frail older adults in Hamilton County and surrounding areas. PACE coordinates health and social services so participants can stay in their own homes, with care delivered at home, at the PACE center and in the hospital when needed. An interdisciplinary team of physicians, nurses, social workers, aides, therapists, dietitians and transportation staff builds an individual care plan for each participant. Reviews are mixed: some praise the team and the care given to loved ones, while one criticizes short therapy sessions and meal portions. It is licensed in Tennessee as an adult day services program.

Services at the PACE center include physician and nursing care, social work, physical, occupational, speech and recreational therapy, nutrition counseling, bathing and grooming help, spiritual care and meals. Transportation and escort to and from the center are provided. The program also covers home care, prescriptions, specialist referrals and other medical services when approved by the PACE team.

The PACE center is at 425 Cumberland St #110, Chattanooga, TN 37404, and is open Monday to Friday, 8:00 AM to 5:00 PM.
