On Lok PACE at Institute on Aging

Institute on Aging (IOA) contracts with On Lok to offer On Lok PACE at IOA, a Program of All-Inclusive Care for the Elderly that helps seniors stay in the community instead of a nursing home. A team of health care professionals creates a personalized care plan for each participant, and services are provided at the center or at home. The plan covers Medi-Cal and Medicare services, including doctor visits, specialists, medications, hospital care, home care, transportation and meals. Google reviews mostly concern IOA as an employer and its drivers rather than the day center itself. It is licensed by the California Department of Public Health as an Adult Day Health Care center.

On Lok PACE covers doctor visits, medical specialists, X-rays, emergency and hospital care, medications and nursing home care, along with home care, transportation and meals. Care is coordinated by a team of health care professionals who create a personalized plan for each participant. Enrolled participants must receive services through On Lok contracted providers except in emergencies.
