Self-Help for the Elderly Adult Day Services

Self-Help for the Elderly's Adult Day Services serves frail seniors and adults with physical or mental impairments or dementia who cannot safely be alone during the day. The program is located at the Jackie Chan Center and aims to help participants stay at home rather than move to a care facility, while giving family caregivers time for work or other obligations. A multi-disciplinary team assesses each participant and creates an individualized care plan. Staff speak English, Vietnamese, Cantonese, Mandarin, Taishanese and other Chinese dialects. It is licensed by the California Department of Public Health as an Adult Day Health Care center.

Services are planned around each person's needs and may include nursing, rehabilitation, dietary counseling, psychiatric and psychological services, and personal care. Social support includes medical social services, counseling and support groups for participants and caregivers. Recreational activities, group exercise, field trips and holiday celebrations fill the day, hot breakfasts and lunches are served, and transportation to and from home is available if needed.

The program is at 408 22nd Ave, San Francisco, CA 94121, and is open Monday through Friday, 9:00 AM to 5:00 PM. New participants need written approval from their primary care physician and proof of tuberculosis clearance within the past 12 months; referrals can come from health providers or families.
