SteppingStone Mabini Day Health

SteppingStone Mabini Day Health is one of three adult day health care centers run by SteppingStone, a San Francisco nonprofit that has provided adult day health care for over 40 years. Participants are older adults and adults with disabilities who have serious chronic health conditions; they attend two to five days a week and receive multiple therapies during each four-hour care shift. Teams of nurses, social workers, physical and occupational therapists and activities staff build an individualized care plan for each person. A reviewer describes group exercise, weigh-ins and fitness coaching, snacks, and many participants arriving by paratransit. It is licensed by the California Department of Public Health as an Adult Day Health Care center.

Each SteppingStone center provides skilled nursing, physical and occupational therapy, social work, activity programming, personal care and nutrition. Services include care coordination, communication with primary care physicians, medication management and monitoring, health assessments and behavioral health screenings. Participants have a hot lunch, and Enhanced Care Management and caregiver support for families of people with dementia are available.
