SteppingStone Mission Creek Day Health

Mission Creek Day Health is one of three adult day health care centers operated by SteppingStone (North & South of Market Adult Day Health), a nonprofit founded in the Tenderloin in 1983 that is now San Francisco's largest adult day health care provider. Participants attend two to five days a week, receiving several therapies during each four-hour care shift, a hot lunch and time with friends. Care teams include nurses, social workers, physical and occupational therapists, activity coordinators and aides, and services are available in multiple languages; SteppingStone also runs Alzheimer's and LGBTQ+ programs and offers Enhanced Care Management. A longtime participant describes kind, warm staff and stimulating, diverse activities, while another reviewer reports an unhelpful interaction. It is licensed by the California Department of Public Health as an Adult Day Health Care center and certified as a Medi-Cal provider.

Each participant has an individualized care plan delivered by a team of nurses, social workers, physical and occupational therapists, activity coordinators and program aides. Services include health assessments, medication management and monitoring, communication with primary care physicians, behavioral health screening, personal care, caregiver support and therapeutic activities. A hot nutritious lunch is served, and SteppingStone supports family caregivers of clients with dementia through a partnership with the Family Caregiver Alliance.

Mission Creek Day Health is at 930 4th St, San Francisco, CA 94158. Hours are Monday through Friday, 7:30 a.m. to 4:30 p.m., and participants attend two to five days a week.
